Help with everyday needs

If you think you need help with your everyday needs to help you to live independently, get in touch with the First Contact Team for an assessment:

The team will complete a referral form on your behalf and then the Community Care Assessment Team will visit you to assess your needs and recommend the best solution.

A home carer can assist with a wide variety of daily living tasks, including personal care and household tasks such as:

  • dressing
  • washing
  • getting to the toilet
  • preparing meals
  • supervising medications
  • doing laundry
  • shopping

Equipment and adaptations

Adaptations to your home to help you to live independently.

Multi-Disciplinary Service and Falls Service

Short term early intervention and prevention to help you remain safely in your home.

Our OneCall service provides assisted living solutions, enabling you to live independently within your own home.

Returning from a stay in hospital

When you are discharged from hospital we will ensure that, if you are eligible, you receive the help you need.
