Stockton-on-Tees Borough Council's Reablement Service provides practical care and support for people discharged from hospital, helping them to live well in their own home while they recover.

The team assess individuals' needs and abilities to do everyday tasks such as get dressed, prepare meals and manage daily life and will set up a care plan to help aid recovery.

The specialist service also helps people to regain skills and confidence to complete errands, assisting them with shopping, washing and getting out and about, as well as gaining new skills to adapt if they need to.

Inspectors rated the service 'Good' following an inspection on 11 May 2021 which focussed on key regulated activities in two areas - safety and leadership.

In their report they recognised that staff "clearly understood" the importance of respecting people's privacy, dignity and independence. They noted that people who use the service "spoke positively about their experiences" and felt their individual needs were met.

Inspectors also praised the safety of service users and staff, including the safe management and administration of people's medication, thorough recruitment and training processes, assessment and management of areas of risk, and infection control procedures.

Councillor Ann McCoy, the Council's Cabinet Member for Adult Social Care, said: "Leaving hospital after an illness or accident can be a daunting experience but staff in the Reablement Service work very hard to make sure people have the support they need to recover safely in the comfort of their own home.

"Not only do they help people get back on their feet but the support offered can also help to avoid unnecessary hospital admission or long-term residential care.

"This rating is a credit to the team and their work to ensure people live independently with confidence. I'd like to congratulate them on this brilliant achievement. I'm sure it's a proud moment for them and I'm delighted the service is supporting our residents at such high standard."
